<title>An open letter to Louisiana&apos;s patients regarding immunizations</title>

Your relationship with your physician should not be politicized. </span></p><p><span style="font-family: Arial;">
As a patient or caregiver in Louisiana, you have a right to have your questions regarding immunizations 
answered in a responsible, physician-led, knowledge-driven, evidence-based manner. The very best person to 
answer your questions is your physician. </span></p><p><span style="font-family: Arial;">
To that end, it is important for patients to have physicians and to be reassured that immunizations remain 
essential to supporting healthy communities. They are one of the most significant medical innovations of our 
time and have proven to be an invaluable thread in the fabric of our society. Traditional vaccinations have 
greatly impacted the spread of (and in some cases nearly eradicated): Polio, Smallpox, Tetanus, Hepatitis B, 
Hepatitis A, Rubella, Hib, Influenza, Measles, Pertussis, Pneumococcal Disease, Rotavirus, Varicella, Diphtheria 
and Mumps. </span></p><p><span style="font-family: Arial;">
In children, vaccinations help to prepare their immune system to recognize and respond to serious diseases. This 
helps them to stay healthy so they can thrive and develop into adulthood. </span></p><p><span style="font-family: Arial;">
Adults are also eligible for vaccines to include boosters for immunizations given during adolescence. In addition 
to childhood shots and/or boosters, adults are eligible to receive: Human Papillomavirus, Pneumonia, Shingles, 
and Meningitis. </span></p><p><span style="font-family: Arial;">
Physicians providing vaccinations to patients do respect a patient’s autonomy and do seek consent. To say 
otherwise is wrong. However, physicians also have a responsibility to provide patients with accurate information 
and the risks associated with refusing a vaccine – including illness, hospitalization and the very real prospect of 
harming vulnerable members of the community who rely on low to non-existent rates of illness to survive. </span></p><p><span style="font-family: Arial;">
Public health should serve as a partner to physicians by promoting evidence-based vaccine policy which is in the 
best interest of the patient. </span></p><p><span style="font-family: Arial;">
Here in Louisiana, physicians are today treating whooping cough (Pertussis), a disease that is highly contagious 
and vaccine preventable. </span></p><p><span style="font-family: Arial;">
In our neighboring state of Texas, physicians are treating an outbreak of Measles, a disease that had been 
declared eliminated in the US in 2000. As vaccination rates drop, outbreaks grow. </span></p><p><span style="font-family: Arial;">

<title>To get politics out of medicine, physicians must get into politics!  </title>

<description><![CDATA[<p><span style="font-size: 12pt; color: #002451;">Now is the perfect time to become engaged in politics as we are changing Governors here in Louisiana. In the last election cycle, we faced a significant turnover in the Legislature. We are again seeing a large turnover in the Legislature, but we will also be changing administrations. When you change administrations, you don't just get a new Governor. You also get a new Commissioner of Administration, a new Health Secretary and a new Executive staff and more.&nbsp;</span><span style="font-size: 12pt; color: #002451;"></span></p> <p><span style="font-size: 12pt; color: #002451;">By nature of the beast, the first year in any new administration lends itself to some chaos. It simply can’t be avoided when you have so many new changes and personalities involved in an adjustment of this scale. While Governor-elect Landry is both astute and sophisticated and hit the ground running, he still won’t have the full power of the office until he is inaugurated on January 8. At that point, he will have to go through the process of Senate confirmations for his selected team. This includes all cabinet appointees, in addition to boards, commissions, task forces, councils and special appointees. As the constitution allows for interim appointments if the legislature is not in regular session, the work of government does not stop. It just means that the Governor and his appointees must be focused on the appointment process while also effecting change.&nbsp;</span><span style="font-size: 12pt; color: #002451;">&nbsp;</span></p> <p><span style="font-size: 12pt; color: #002451;">Shifting to the Legislature shows us 40 new members who were elected this Fall. This count addresses only those individuals who have never served, not those who moved from one chamber to the other. This means over 25% of the Legislature is new. This election also saw both the Senate President and The House Speaker termed out – meaning new leaders for both bodies. It is expected that Sen. Cameron Henry will be the Senate President and Rep. Phillip DeVillier will be the House Speaker. What comes next will be selections for both Pro Tem positions and then new committee chairs and vice chairs will be appointed.&nbsp;&nbsp;</span><span style="font-size: 12pt; color: #002451;"></span></p> <p class="xelementtoproof"><span style="font-size: 12pt; color: #002451;">Put your running shoes on and get ready for the round robin to come. There are people to meet, legislators to educate and chaos to overcome! We are already planning White Coat Wednesday and will be announcing that date early in January. Please make plans to join us this year as every voice is important!&nbsp;</span></p>]]></description>

<title>Ask what you can do for your Profession!</title>

As the Chair of the Council on Legislation for the Louisiana State Medical Society, I want to start by thanking you, the members, for responding to our ask for your participation and for making your voice heard in Baton Rouge this past year. Early in the session, we hosted the 2022 Legislative Reception and White Coat Wednesday events. While the details of these events will be chronicled elsewhere, suffice it to say that for two years in a row, we have “outgrown the facility” based upon your turnout and willingness to engage. With well over 120 members in attendance this year, there can be no doubt that our society is experiencing a new “finest hour” of professional advocacy. </p> <p>Next to thank is your Council on Legislation. This is the committee or “council” of the society, made up of individual members (one from each District), who work year-round to ensure success when the legislative session arrives. In addition to attending the fifteen plus meetings we hold every year, they offer strategy and insights into key issues and relationships at the Capitol based, in many cases, on decades of experience in the Legislative process. We do Sunday evening conference calls throughout session in addition to the three or so other meetings we hold on your behalf each year. You would be incredibly proud of your fellow members if you witnessed the commitment they bring to the table in this effort. They are truly advocacy warriors for the profession!</p> <p>I would also be remiss if I didn’t highlight the incredible work of our LSMS Governmental Affairs team, especially Maria Bowen, our Vice President of Governmental Affairs and Lauren Bailey, our Vice President of Legal Affairs. They work tirelessly, year-round, but especially during session, to ensure your objectives, as described in LSMS policy, are achieved. Maria has completed her fourth session now with the LSMS, but it is hard to imagine where we would be without her. When she joined, we discussed how priority number one would be to re-engage the grassroots membership of the LSMS. I am comfortable, based upon the success of White Coat Wednesday, in saying that this crucial first step is achieved.</p> <p>I do get asked by members throughout the year what they can do to help become better advocates for the profession. There is no question that the answer is to form a relationship with your individual state Senator and Representative and support them politically. Starting such a relationship is easiest to accomplish with a first-time candidate, but in my experience, even established elected officials are surprisingly receptive if you reach out to them outside of the time-crunch of the legislative session. Many of you are already aware of the members from growing up together or interacting casually in your personal lives, so that makes the introductions easier. It also helps if the first time you are reaching out to them, you simply introduce your profession along with some of your “feel good” priorities (making telemedicine better or helping with the opioid epidemic, as examples) and don’t have a specific “ask.” This budding relationship will be developed even further when you have the opportunity to work on or contribute to their next campaign. These efforts don’t bring a direct return, but will help you in getting their attention during the busy season of session and at least allow you the opportunity to articulate your position on ways to help improve legislation they are considering. </p> <p>Don’t hesitate to approach me at the upcoming LSMS House of Delegates in August if you have questions and want to get more involved in professional advocacy. We will no doubt face big challenges in the coming years and we will need you at the table with us!</p>]]></description>

<title>State&apos;s Largest Physician PAC Endorses Candidates</title>

<description><![CDATA[<p style="text-align: justify;">The Louisiana Medical Political Action Committee (<span class="scayt-misspell-word" data-scayt-word="LAMPAC" data-wsc-lang="en_US">LAMPAC</span>), the political arm of the Louisiana State Medical Society (<span class="scayt-misspell-word" data-scayt-word="LSMS" data-wsc-lang="en_US">LSMS</span>), has endorsed candidates in the Governor's race, 35 legislative races, and in the East Baton Rouge Parish Coroner's race. <span class="scayt-misspell-word" data-scayt-word="LAMPAC" data-wsc-lang="en_US">LAMPAC</span> is a non-partisan, state political action committee. It is a separate organization from <span class="scayt-misspell-word" data-scayt-word="LSMS" data-wsc-lang="en_US">LSMS</span> and is responsible for political fundraising and candidate support.<br />

"<span class="scayt-misspell-word" data-scayt-word="LAMPAC" data-wsc-lang="en_US">LAMPAC</span> is pleased to offer our support to these endorsed candidates, three of whom are <span class="scayt-misspell-word" data-scayt-word="LSMS" data-wsc-lang="en_US">LSMS</span> members and physicians," Robert Bass, MD, <span class="scayt-misspell-word" data-scayt-word="LAMPAC's" data-wsc-lang="en_US">LAMPAC's</span> chairman. "It's been a number of years since we decided to take a position in the Governor's race, but we have one of our own running in Dr. Ralph Abraham. We believe that physicians supporting physicians is important, and when you have a candidate like Dr. Ralph Abraham, it's very easy to make this decision. Dr. Abraham has a wealth of knowledge that is critical to helping Louisiana improve healthcare outcomes. In its first round of endorsements, the PAC has endorsed 37 candidates. The PAC is continuing to review other races and candidates for viability in alignment with its goals and objectives.<br />

<title>Louisiana selects four companies to manage care for state&apos;s 1.7 million Medicaid enrollees</title>

<p style="text-align: justify;">The Louisiana Department of Health has picked four companies it intends to award multi-billion dollar contracts to in exchange for managing care for 1.7 million Medicaid patients in the state.<br />
<br />
The agency picked AmeriHealth Caritas Louisiana, Community Care Health Plan of Louisiana (Healthy Blue), Humana Health Benefit Plan of Louisiana and United Healthcare Community Plan. The terms of the deals are still being worked out but are expected to begin in January.<br />
<br />
Managed care companies are tasked with providing Medicaid benefits and services to the 1.7 million adults and children who are covered by the state-sponsored insurance program, which was expanded by Gov. John Bel Edwards to cover nearly half a million more people.<br />
<br />
Currently, the state contracts with five companies to manage services for most of the state Medicaid population. Those companies are Aetna Better Health, AmeriHealth Caritas Louisiana, Healthy Blue, Louisiana Healthcare Connections and United Healthcare Community Plan.<br />
<br />
Medicaid paid $7.6 billion to those firms in the 2018 fiscal year. The health department forecast it would spend more than $8 billion in the fiscal year that ended June 30 – including $4.7 billion for regular Medicaid patients and $3 billion for expansion patients.<br />
<br />
The existing contracts expire at the end of the year. They were negotiated by the Jindal administration and extended by the Edwards administration.<br />
<br />
The health department began a bid process in February for the new contracts.<br />
<br />
Former Gov. Bobby Jindal moved the state to the privatized managed care model in 2012, moving away from the old system of directly reimbursing doctors and hospitals for each service for Medicaid patients.<br />
<br />
Now, most Medicaid enrollees receive their health care through the new managed care model, though not long-term supports and services. Medicaid pays the managed care organizations, often called MCOs, a monthly fee based on the number of enrollees.<br />
<br />
If a current Medicaid enrollee has a plan with one of the MCOs that was not selected for the new round of contracts, the enrollee may have to select a new plan, the health department said. The open enrollment process begins in October where members can select new health plans.<br />
<br />
The contracts are expected to be executed later this month.</p>

<title>LSMS Request - End Prior Authorization for Opioid Treatment in Louisiana</title>

The Louisiana State Medical Society (LSMS) strongly urges the State of Louisiana to follow in the footsteps of Pennsylvania in removing prior authorization requirements for treatment of substance use disorders.  We believe and support all initiatives which will result in streamlining the efforts of saving lives in Louisiana.  Louisiana can and should serve as a leader in this effort.</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
We ask that you take immediate action to remove the barriers of such by eliminating prior authorization requirements relative to opioid treatment in Louisiana. </span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
Governor Wolf of Pennsylvania and his administration announced Friday, October 12, 2018, a landmark agreement with the seven major payers in the Commonwealth to end prior authorization for medication-assisted treatment as well ensure that medication-assisted treatment (MAT) is on the lowest cost-sharing tier. Highlights from the announcement can be found below:</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
“It is vital we take all possible steps to make sure patients are receiving the most appropriate treatment for their pain, while at the same time appropriately managing and monitoring the risks associated with opioids,” Department of Health Secretary Dr. Rachel Levine said. “Medication-assisted treatment is an effective, evidence-based treatment to help those with the disease of addiction to opioids and this step by private insurers allows more people with opioid use disorder to be able to access this form of treatment. Treatment works, and recovery is possible for those who are battling this disease.”</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
These guidelines apply to individual, small group, and large group fully insured plans. Self-funded plans, where employers provide health care coverage administered by a third party, are regulated by the federal government and are not included in this agreement.</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
The guidelines implement thresholds for prior authorization for long- and short-acting opioids, morphine milligram equivalents (MME) and exceptions for active cancer, sickle cell crisis, and palliative care/hospice patients. Generally, commercial insurers are requiring prior authorization for all long-acting opioid prescriptions and short-acting opioid prescriptions after seven days. Some insurers are phasing in their alignment with many of the guidelines. Patients should consult with their insurer to find out how these guidelines are being incorporated into their specific health plan.</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
Providing unrestricted access to all commonly used forms of MAT when patients need it is critical to ensuring any individual struggling with addiction can access the type of treatment that is best for them. Under this agreement, commercial insurers will cover MAT without prior authorization in the following ways:</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
•	Coverage of at least one Buprenorphine/naloxone combination product<br />
•	Coverage of Methadone as MAT<br />
•	Coverage of injectable and oral Naltrexone<br />
<br />
Commercial insurers have also committed to coverage of at least one form of nasal naloxone without quantity limits.</span></p>
<p style="margin-bottom: 0.0001pt; text-align: justify;"><span style="color: black;"><br />
The guidelines also provide that MAT will be covered at the lowest patient cost tier on the plan’s pharmacy benefit, as applicable.</span></p>

Once again, the LSMS implores your leadership in the State of Louisiana to take immediate action to follow in the footsteps of Pennsylvania in removing prior authorization requirements for treatment of substance use disorders.</span></p>

<p><span style="color: #000000;">This Tuesday the Senate Health, Education, and pensions committee will have a hearing on the opioid crisis and an examination of cause and effects. On Thursday they will have an&nbsp;</span><i><span style="color: black;">Executive Session</span></i><span style="color: black;"> to Consider the Nominations of Brigadier General Mitchell Zais to be Deputy Secretary of the Department of Education, Kenneth Marcus to be Assistant Secretary for Civil Rights of the Department of Education, James Blew to be Assistant Secretary for Planning, Evaluation, and Policy Development at the Department of Education, Patrick Pizzella to be Deputy Secretary of the Department of Labor, Scott Mungo to be Assistant Secretary of Labor for Occupational Safety and Health, Cheryl Stanton to be Administrator of the Wage and Hour Division at the Department of Labor, Dr. William Beach to be Commissioner of Labor Statistics at the Department of Labor, Dr. Brett Giroir to be Assistant Secretary for Health at the Department of Health and Human Services, and possible additional re-nominations.</span></p>
